The Centers for Medicare and Medicaid Service give a general rating to the nursing facility, and also rate health inspections, staffing, and Quality Measures. The Medicare criteria for facilities are based on both state and federal data. For example, a skilled nursing facility that achieves 4 Stars on health inspections has had less citations than average on their state health inspections.
